I am aware that this program requires a 3-5 hour per month commitment.

I agree to come to each mentor meeting prepared by having read the agenda beforehand and preparing any notes that are needed.

I am aware that I need access to some form of video chat software (Zoom, Teams, Skype, WebEx, Hangouts, Jitsi) to participate in this program (most of these services are free but you will need to create an account if you don't have one currently).
